The rope was clipped into the quickdraw, but it was clipped backwards. At a glance everything looked right. The rope was in. The draw was clipped. We were moving upward. But orientation matters. Back clipping means the rope runs through the carabiner in the wrong direction. In a fall, the rope can pull across the gate and unclip itself.
