Neglected pathological neck femur fracture in a 16 year old male. We fixed it with a sliding/dynamic hip screw (SHS/DHS) while ensuring that biology/biomechanics (alignment and stability) remain preserved. Minimising rhe use of drilling and avoiding too many holes in proximal fragment and accepting alignment over anatomical reduction is key to success in our hands. Some kind of loading/ weight bearing should be promoted for early bony remodelling. Here go remodelling can be seen in follow up rad
